Bone is better imaged by conventional x-rays in some cases and CT Scans are preferred for patients with severe bleeding. A MRI may not always distinguish between tumor tissue and edema fluid and does not detect calcium when this is present within a tumor. Magnetic Resonance Imaging (MRI) Information

For your magnetic resonance imaging exam, the technologist will position you on a table that will slide into the scanner.  With the newest technology it takes only a few minutes, and unlike the older technology it is open.  A magnetic resonance imaging technologist will maintain constant communication with you throughout the exam. The resulting images will be reviewed and interpreted by a radiologist and a report will be sent to your doctor.

Because of the magnetic field, you will be asked to wear metal-free clothing.  You also will be asked to remove any metallic objects, such as jewelry, watches and hair clips.  Inform your technologist of prior surgeries or metal implants, such as pacemakers or aneurysm clips.

Difference between CT's, X-rays and MRI's...Unlike x-rays and CT's, MRI's do not use radiation.  Instead, magnetic resonance creates high-quality images through the combination of a strong magnetic field and radio waves.  MRI's can detect certain diseases much earlier than other medical imaging techniques, making it the diagnostic tool of choice for many physicians.

Body parts the MRI can evaluate...Physicians use the MRI scanner to examine one part of the body at a time.  The scanner can take pictures of the head, neck, back abdomen, pelvis, shoulder, elbow, knee, blood vessels and more.

Preparations...People having IV sedation or contrast (dye) should not eat for several hours prior to the test. Talk to the center to get specific instructions.

Risks...MRI's are considered safe.  There are no known health risks associated with the magnetic field or the radio waves used by the machine nor should there be any side effects.

Pregnant or breastfeeding...MRI's are not recommended for pregnant women unless it is medically indicated.  If you are breastfeeding and have contrast as part of your exam, please suspend nursing for 24 hours after the exam.

Noisy...Scanners work with strong metallic fields, which builds up energy.  The energy is released as loud knocking sounds.

How should I prepare for the MRI procedure?

In most cases surgical staples, plates, pins and screws pose no risk during MRI if they have been in place for more than four to six weeks. Tattoos and permanent eyeliner may also create a problem. You will be asked if you have ever had a bullet or shrapnel in your body or ever worked with metal. If there is any question of metal fragments, you may be asked to have an x-ray that will detect any such metal objects. Tooth fillings usually are not affected by the magnetic field but they may distort images of the facial area or brain, so the radiologist should be aware of them. The same is true of braces, which may make it hard to "tune" the MRI unit to your body. You will be asked to remove anything that might degrade MRI images of the head, including hairpins, jewelry, eyeglasses, hearing aids and any removable dental work.

The radiologist or technologist may ask about drug allergies and whether head surgery has been done in the past. If you might be pregnant, this should be mentioned. Some patients who undergo MRI in an enclosed unit may feel confined or claustrophobic. If you are not easily reassured, a sedative may be administered. Roughly one in 20 patients will require medication to reduce the anxiety associated with claustrophobia.

What does the MRI equipment look like?

The conventional  MRI unit is a closed cylindrical magnet in which the patient must lie totally still for several seconds at a time and consequently may feel "closed-in" or truly claustrophobic. However, new "patient-friendly" designs are rapidly coming into routine use.

The "short-bore" systems are wider and shorter and do not fully enclose the patient. Some newer units are open on all sides, however the image quality may vary.

How is the procedure performed?

The patient is placed on a sliding table and positioned comfortably for the MRI examination. Then the radiologist and technologist leave the room and the individual MRI sequences are performed. The patient is able to communicate with the radiologist or technologist at any time using an intercom. Also, many MRI centers allow a friend or, if a child is being examined, a parent to stay in the room. Depending on how many images are needed, the exam will generally take 15 to 45 minutes, although a very detailed study may take longer. You will be asked not to move during the actual imaging process, but between sequences some movement is allowed. Patients are generally required to remain still for only a few seconds to a few minutes at a time.

Depending on the part of the body being examined, a contrast material (usually gadolinium) may be used to enhance the visibility of certain tissues or blood vessels. A small needle connected to an intravenous line is placed in an arm or hand vein. A saline solution will drip through the intravenous line to prevent clotting until the contrast material is injected about two-thirds of the way through the exam.

When the exam is over the patient is asked to wait until the images are examined to determine if more images are needed. A radiologist experienced in MRI will analyze the images and send a report with his or her interpretation to the patient's personal physician. This should take only a few days or less.

What will I experience during the MRI procedure?

MRI causes no pain, but some patients can find it uncomfortable to remain still during the examination. Others experience a sense of being "closed in," though the more open construction of newer MRI systems has done much to reduce that reaction. You may notice a warm feeling in the area under examination; this is normal but if it bothers you the radiologist or technologist should be notified.

If a contrast injection is needed, there may be discomfort at the injection site and you may have a cool sensation at the site during the injection. Most bothersome to many patients are the loud tapping or knocking noises heard at certain phases of imaging. Ear plugs may help.

Who interprets the results and how do I get them?

A radiologist, who is a physician experienced in MRI and other radiology examinations, will analyze the images and send a signed report with his or her interpretation to the patient's personal physician. The patient receives MRI results from the referring physician who ordered the test. New technology also allows for distribution of diagnostic reports and referral images over the Internet at many facilities.

What are the limitations of MRI of the body?

Bone is better imaged by conventional x-rays in some cases and CT is preferred for patients with severe bleeding. MRI may not always distinguish between tumor tissue and edema fluid and does not detect calcium when this is present within a tumor. In most cases the examination is safe for patients with metal implants, with the exception of a few types of implants, so patients should inform the technician of an implant prior to the test. The examination must be used cautiously in early pregnancy. MRI typically costs more than CT scanning.

What is a CAT scan?

A computerized axial tomography scan is more commonly known by its abbreviated name, CAT scan or CT scan. It is an x-ray procedure which combines many x-ray images with the aid of a computer to generate cross-sectional views and, if needed, three-dimensional images of the internal organs and structures of the body. A CAT scan is used to define normal and abnormal structures in the body and/or assist in procedures by helping to accurately guide the placement of instruments or treatments. A large donut-shaped x-ray machine takes x-ray images at many different angles around the body. These images are processed by a computer to produce cross-sectional pictures of the body. In each of these pictures the body is seen as an x-ray "slice" of the body, which is recorded on a film. This recorded image is called a tomogram. "Computerized Axial Tomography" refers to the recorded tomogram "sections" at different levels of the body.

Why are CAT scans performed?

CAT scans are performed to analyze the internal structures of various parts of the body. This includes the head, where traumatic injuries, (such as blood clots or skull fractures), tumors, and infections can be identified. In the spine, the bony structure of the vertebrae can be accurately defined, as can the anatomy of the intervertebral discs and spinal cord. In fact, CAT scan methods can be used to accurately measure the density of bone in evaluating osteoporosis..

Occasionally, contrast material (an x-ray dye) is placed into the spinal fluid to further enhance the scan and the various structural relationships of the spine, the spinal cord, and its nerves. CAT scans are also used in the chest to identify tumors, cysts, or infections that may be suspected on a chest x-ray. CAT scans of the abdomen are extremely helpful in defining body organ anatomy, including visualizing the liver, gallbladder, pancreas, spleen, aorta, kidneys, uterus, and ovaries. CAT scans in this area are used to verify the presence or absence of tumors, infection, abnormal anatomy, or changes of the body from trauma.

The technique is painless and can provide extremely accurate images of body structures in addition to guiding the radiologist in performing certain procedures, such as biopsies of suspected cancers, removal of internal body fluids for various tests, and the draining of abscesses which are deep in the body. Many of these procedures are minimally invasive and have markedly decreased the need to perform surgery to accomplish the same goal.

Are there risks in obtaining a CAT scan?

A CAT scan is a very low-risk procedure. The most common problem is an adverse reaction to intravenous contrast material. Intravenous contrast is usually an iodine-based liquid given in the vein, which makes many organs and structures, such as the kidneys and blood vessels much more visible on the CAT scan. There may be resulting itching, a rash, hives, or a feeling of warmth throughout the body. These are usually self-limiting reactions and go away rather quickly.

Toxicity to the kidneys which can result in kidney failure is an extremely rare complication of the intravenous contrast used in CAT scans. Diabetics, dehydrated individuals, or patients who already have impaired kidney function are most prone to this reaction. Newer intravenous contrast agents have been developed, such as Isovue, which have nearly eliminated this complication.

The amount of radiation a person receives during a CAT scan is minimal. In men and non-pregnant women, it has not been shown to produce any adverse effects. If a woman is pregnant, there may be a potential risk to the fetus, especially in the first trimester of the pregnancy. If a woman is pregnant, she should inform her doctor of her condition and discuss other potential methods of testing, such as an ultrasound, which are not harmful to the fetus.

How does a patient prepare for CAT scanning, and how is it performed?

In preparation for a CAT scan, patients are often asked to avoid food, especially when contrast material is to be used. Contrast material may be injected intravenously, or administered by mouth or by an enema in order to increase the distinction between various organs or areas of the body. Therefore, fluids and food may be restricted for several hours prior to the examination. If the patient has a history of allergy to contrast material (such as iodine), the requesting physician and radiology staff should be notified. All metallic materials and certain clothing around the body are removed because they can interfere with the clarity of the images.

Patients are placed on a movable table, and the table is slipped into the center of a large donut-shaped machine which takes the x-ray images around the body. The actual procedure can take from a half an hour to an hour and a half. If specific tests, biopsies, or intervention are performed by the radiologist during CAT scanning, additional time and monitoring may be required. It is important during the CAT scan procedure that the patient minimize any body movement by remaining as still and quiet as is possible. This significantly increases the clarity of the x-ray images. The CAT scan technologist tells the patient when to breathe or hold his/her breath during scans of the chest and abdomen. If any problems are experienced during the CAT scan, the technologist should be informed immediately. The technologist directly watches the patient through an observation window during the procedure and there is an intercom system in the room for added patient safety.

CAT scans have vastly improved the ability of doctors to diagnose many diseases earlier in their course and with much less risk than previous methods. Further refinements in CAT scan technology continue to evolve which promise even better picture quality and patient safety. Newer CAT scans called "spiral" or "helical" CAT scans can provide more rapid and accurate visualization of internal organs. For example, many trauma centers are using these scans to more rapidly diagnose internal injuries after serious body trauma.

How does the MRI procedure work?

MRI is a unique imaging method because, unlike the usual radiographs (x-rays) or even Computed Tomography (CT) scanning, it does not rely on radiation. Instead radiofrequency waves are directed at protons, the nuclei of hydrogen atoms, in a strong magnetic field. The protons are first "excited" and then "relaxed," emitting radio signals that can be computer-processed to form an image. In the body, protons are most abundant in the hydrogen atoms of water—the "H" of H2O—so that an MR image shows differences in the water content and distribution in various body tissues. Even different types of tissue within the same organ, such as the gray and white matter of the brain, can easily be distinguished. Typically an MRI examination consists of two to six imaging sequences, each lasting two to 15 minutes. Each sequence has its own degree of contrast and shows a cross-section of the body in one of several planes (right to left, front to back, upper to lower).

Computerized axial tomography (CAT) scan machines produce X-rays, a powerful form of electromagnetic energy. X-ray photons are basically the same thing as visible light photons, but they have much more energy. This higher energy level allows X-ray beams to pass straight through most of the soft material in the human body.  The computer takes all this information and puts together a 3-D image of the body.

Magnetic Resonance Imaging (MRI Scan) Glossary

Brain: That part of the central nervous system that is located within the cranium ( skull ). The brain functions as the primary receiver, organizer and distributor of information for the body. It has two (right and left) halves called "hemispheres."
Contrast: Short for "contrast media." Contrast media are x-ray dyes used to provide contrast, for example, between blood vessels and other tissue.
Heart: The muscle that pumps blood received from veins into arteries throughout the body. It is positioned in the chest behind the sternum (breastbone; in front of the trachea, esophagus, and aorta; and above the diaphragm muscle that separates the chest and abdominal cavities. The normal heart is about the size of a closed fist, and weighs about 10.5 ounces. It is cone-shaped, with the point of the cone pointing down to the left. Two-thirds of the heart lies in the left side of the chest with the balance in the right chest.
Magnetic resonance imaging : A special radiology technique designed to image internal structures of the body using magnetism, radio waves, and a computer to produce the images of body structures. In magnetic resonance imaging (MRI), the scanner is a tube surrounded by a giant circular magnet. The patient is placed on a moveable bed that is inserted into the magnet. The magnet creates a strong magnetic field that aligns the protons of hydrogen atoms, which are then exposed to a beam of radio waves. This spins the various protons of the body, and they produce a faint signal that is detected by the receiver portion of the MRI scanner. A computer processes the receiver information, and an image is produced. The image and resolution is quite detailed and can detect tiny changes of structures within the body, particularly in the soft tissue, brain and spinal cord, abdomen and joints.

Stroke : The sudden death of some brain cells due to a lack of oxygen when the blood flow to the brain is impaired by blockage or rupture of an artery to the brain. A stroke is also called a cerebrovascular accident or, for short, a CVA.
